Pre hospital discharge assessments

Better and timely assessments prior to discharge and better discharge planning.

Why the contribution is important

This should reduce stress to both the patient and his/her family by avoiding very short notice (e.g. 2 hours) of disharge due to bed pressure in the hospital.  Better assessments would also better gauge whether the patient could function well at home and avoid, for example, an inability to go upstairs (only after 3 weeks with re-enablement support), inability to leave the houseb (only after a further 3 weeks) and an inability to make own meals (after 6 weeks).   Assessments, especially for the elderly, should not merely ask "Can you take a few steps?" but rather "Can you look after yourself on your own?".
